Consultation Request in Person - GAA

For requests to consult your Claims History Statement, we invite you to use the Online consultation with clicSÉQUR authentification or the Online consultation services rather than make your request in person.

To obtain a copy of your Claims History Statement in person, please follow the steps below.

Please note that Claims History Statements can not be consulted in person without an appointment.

An agent will contact you within 48 hours.

On the day of your appointment, go to GAA’s office.

You'll be asked to complete the Consultation Request form and to show your driver's license.

An agent will then hand you a copy of your Claims History Statement.
